A-       For the first two weeks you should sleep on your stomach and not on your back. During this time you should avoid putting direct pressure on your buttock. If you have to sit, use a very soft cushion. From week three until six you should also sit on cushions when sitting for prolonged periods.

 

Q- 30  Will I experience any nauseas during the buttock implant recovery process?

 

A-      Yes, it may occur. Make sure that you have all your medications, cold drinks, and towels at your bedside. Most people are nauseated during the first 24-48 hours. Take a nausea pill one hour before taking your pain pills or antibiotics. While it is good to drink plenty of liquids, refrain from drinking plain water or acidic fruit juices as they will make you more nauseated.

 

Q-31   Will I experience any numbness in the surgical areas?

 

A-       Yes. All surgical areas may stay numb for four to eight weeks and in some rare cases up to 6 months. Lymphatic Massages will help to desensitize your skin in order to avoid hypersensitivity or skin irritation.

 

Q-32   After the buttock implant procedure is done is it normal some blood to drip from the incisions?

 

A-      Do not be alarmed when blood-tinged tumescent fluid oozes from the sites of fat harvest or liposuction. This is a normal result of liposuction and usually stops during the second week. The plastic surgeon leaves these tiny incisions exposed so that the liquid spills out, thereby minimizing bruising. You should cover your bed to avoid staining from this fluid. There should not be any drainage from the incision where the implant has been placed. If this happens, please inform your doctor.

A-      Yes. There will be some swelling and some minimal bruising which dissipates five to eight weeks following surgery.

A-      Yes. Following liposuction, fat injection, or implant placement, you are placed in a support garment (girgle). Do not remove this until your doctor instructs you to do so. Support garments should be worn 24 hours a day for two months, and then during the daytime for the next three weeks. This will help to reduce swelling, and to help shrink the skin, and to shape the buttock area.

 

Q-35 Will I be allowed to remove the garments for showering after surgery?

 

A-      Usually the patient removes their garment and takes a shower after the second day.

 

Q-36   When I take shower what should I do? Can I use some moisturizing lotion in the wounded areas?

 

A-      The first time that you remove your garment you will feel like you are going to faint. This is normal. Have someone with you at that time. After your first shower you need to apply Vaseline or heavy lotion all over liposuctioned areas and the buttock skin to elevate the itchiness that is caused by the skin-drying effects of liposuction. This is a good time for you to start massaging the fat injection areas. You should massage these areas softly about three times a day for about six weeks. Sometimes, patients whose skin was not firm and elastic will experience dimpling of the skin. This can be eliminated or minimized by massaging and early exercise. If you have implants don’t get the incision wet until instructed by your surgeon.

 

Q-37 What are others general care that I should learn on recovery?

 

A-      The most important way that a person can contribute to having a great result is by being meticulous about conducting massage.

You need to meticulously clean your buttock implant incision in the shower and then after with alcohol after each bowl movement, or at least once daily for the first two weeks after surgery.

 

Patients will also feel a bruise-like or muscle cramp-like pain, particularly in the areas where fat was removed, and which will be addressed through a prescription from your surgeon.

Antibiotics are prescribed to reduce the risk of infection.

 

It is good to ambulate after surgery in order to prevent stagnation of blood in your legs and to reduce the chance of developing blood clots.

 

Q-38   When Can I Resume Work and Exercise after having a Brazilian butt lift or buttocks implants?

 

A-      Most patients can resume normal activities within two weeks after buttock implant placement, or within one week after fat grafting. After this time you can start gentle walking but must refrain from jogging or doing heavy physical activity or exercising until six weeks after surgery. You can do upper body exercising after three weeks. Most patients return to work within a week after fat grafting and after two weeks from buttock implant placement.

A-      Fat Injection does not hinder your ability to have sexual relations. However, you should resume sexual activity only as you feel comfortable in doing so. Q-42   Who Are The Best Candidates for Buttock Implants?

 

A-        The buttocks region has proven to be an area of struggle for many people. It is not easily enlarged or shaped by weight training and diet. If enlargement of the buttock is ones goal, there are a variety of different size and shape buttock implants from which to choose. The decision for the shape and size of the buttock implants is made depending on each person’s individual desires and on their general body build. In general, the best candidates for buttock augmentation are:

• 18 years of age or older
• In good physical health
• Psychologically stable
• Wanting to improve their appearance
• Realistic in their expectations
• Having this surgery for the first time

A-          Buttock implants are made of silicone as are breast implants, but they differ in consistency. Breast implants are soft and fluid filled, while the buttock implants are soft but made of a solid silicone. Under general anesthesia, the silicone implants are placed into each buttock area through a single incision overlying the tailbone. The buttock implants are positioned to look as natural as possible for the optimal cosmetic effect.

With the patient asleep under general anesthesia, a two to three inch incision is made between the buttocks in the midline. Through this incision the buttock muscle, or gluteus maximus, is lifted up and a pocket is made just large enough for the implant. The buttock implant is inserted into its pocket. The opposite side is then completed. Both sides are carefully examined in order to assure that the results look natural and symmetric. Dissolvable stitches are used to close the incision. A compression bandage is placed around the buttocks to help reduce discomfort and swelling. The taped dressing that is applied during surgery to provide compression and support is usually removed three to four days following surgery. At this time the patient can take showers and begin to move around more normally. The results can be seen immediately but the buttocks look more natural and become softer as the muscle stretches during the several months after surgery.

 

Q-44   What can I expect from buttock implants?

 

A-        There is generally very little swelling and bruising after buttock augmentation so the results may be seen very soon after surgery. Unfortunately since this is an area of the body that is required for walking, sitting, and many other movements, patients might experience greater pain during the recovery period than other common cosmetic surgery procedures and the recovery time is usually longer. Pain medications typically are prescribed to help with the discomfort, but after approximately 5-7 days the patient is able to be up and about; moving, walking, and sitting more comfortably. Full physical activities are usually permitted after 20 days after surgery. It is important to take the medication prescribed to you by your doctor. Q-57   How can I Prepare for Buttock Augmentation procedure?

 

A-      The plastic surgeon will provide very specific instructions prior to surgery so you will have an optimal and safe experience. This includes what medications to avoid to prevent excessive bleeding and bruising, required medical exams, what to eat and drink prior to surgery, and the prescription of an anti-nausea pill to take on the morning of the surgery. In all cases, you will be asked to have a reliable friend or family member take you home after the surgery, as you will not be allowed to drive.

If you are going to have buttock implants you are not allowed to sit on your buttocks for more than a few minutes at a time during, the first two weeks. This means no driving during this period. From the third to sixth week after surgery, you can only sit on soft pillows and cushions. Make sure that you have enough pillows and cushions to help you during this period.

Because the incision is located in the buttock crease, it needs to be meticulously cleaned in the shower at least once daily and every time after bowel movement, during the first two weeks. Therefore, make preparations for this.

 

Q-58   What are the pre-operative instructions I should observe for buttock implants?

 

A-

• A- If you are older than 45 years old, or have heart disease, we require a pre-operative EKG, which would either be done by your internist or cardiologist. Heart disease and all other medical illnesses need to be monitored first by an internist or specialist.
• If you are anemic, or have a low blood count, please notify the office well in advance of surgery so that they can start you on Iron supplement. You cannot have surgery if you are anemic.
• Pre-operative laboratory blood and urine tests are done usually within 10 days of your surgery date. If you do have health insurance this might be covered, otherwise, the laboratory will charge you.
• If you have specific medical illnesses, allergies, or physical handicaps, please notify the staff during your pre-operative visit. Please refrain from taking any Aspirin, Aleve, Advil, Motrin or other NSAID for ten days before your surgery. These drugs can increase the incidence of bleeding and bruising.
• Do not drink or eat after midnight, the night before your surgery. This means no coffee or breakfast in the morning of your surgery. You should take all your medications with a little water.
• Please remove and leave behind all jewelry, watches, and body rings before you come in for your surgery. Please do not wear any makeup or perfume.
• Before leaving home from your surgery, take one “nausea pill”- COMPAZINE – with a little water.
• Please wear comfortable and loose clothing the day of surgery. It would be helpful if you wear front open tops and loose sweat pants. Check the fit of your compression garment in advance and bring it with you to surgery.
• You should make arrangements for a responsible adult friend or family member to take you home after surgery. You are not allowed to drive yourself home or take a taxi. After surgery, this caregiver is required to stay with you and to monitor and assist you for at least the first 48 hours after surgery. 

Q- 59   What Are the Major Risks and Potential Complications of Buttock Augmentation?

 

A-      Like any non-cosmetic major surgery, buttock augmentation has potential risks and complications. Therefore, you should consider it as seriously as you would any major surgery. Also note that augmentation with implants carries a much higher rate of complications as compared to fat injections.

 

Q-60   What Are the Common Minor Complications, or Consequences?

 

A-        Changes in Skin Sensation: This is a normal consequence that occurs in everyone to a different extent. You may experience “paresthesia,” which is an altered sensation at the site of the liposuction. This may either be in the form of an increased sensitivity (pain) in the area, or the loss of any feeling (numbness) in the area. This sensation is permanent in very rare cases. However, in almost all patients it goes away within the first two to four months after surgery. Massaging the area of liposuction helps increase circulation, and facilitates return of normal skin sensation.

 

Swelling: This is also a normal occurrence that occurs in every patient. Compression garments and post-operative massage help to minimize this, as well as reducing its duration to less than two months.

 

Bruising: There will be bruising in many areas that are liposuctioned or grafted, as well as in adjacent or dependent areas. For example, you will also note bruising and swelling in the back of your thighs and your genitalia. This is not because these areas are actually liposuctioned. However, bruises usually move downward due to gravity, and can thus temporarily accumulate in the genitalia. Some bruising can last a long time or even lead to permanent skin pigmentation. Sun tanning immediately after surgery can cause these bruises to become permanent skin stains.

 

Skin Irregularities or Dimples: The areas that are injected with fat can develop contour irregularities, including bumps and dimples depending on your skin elasticity and the extent of fat injection. Usually, post-operative massage helps to smooth these areas.

Asymmetry: No two halves of the body are identical. Therefore, following buttock augmentation, there may appear to be very slight differences between the contour and size of the two sides. Scoliosis or other spine derangements can magnify body asymmetry.

 

Muscle Spasm: It is very common to get temporary muscle spasm or cramps in the buttock muscles. This usually resolves quickly by itself or can be helped by gentle heat and massage.

 

Pain and Discomfort: You should expect to have pain or discomfort similar to a feeling of having worked out too hard. Some of this may feel like a stabbing pain in the augmented areas, and in adjacent areas. This can usually be managed by pain medications, and it generally lasts for a few weeks. Rarely, and in people with Fibromyalgia, or other forms of chronic pain syndrome, there can be a worsening of the patient’s baseline pain.

 

Depression, emotional, or sexual changes: There have been reported incidents of post-liposculpture depression similar to postpartum depression. While this is usually temporary, patients need to address it with their physician.

 

Seroma or fluid collection: After fat injection or implant placement, there may be a pooling of serum, the straw colored liquid from your blood, in the buttock areas. Your surgeon can alleviate these by draining them with a small needle in an office procedure.

 

Fat Absorption and Sclerosis: Approximately 30% of the injected fat does not survive and gets absorbed. If this happens unevenly, there could be areas were you feel or see dimples or dents. Also, in the areas of fat absorption scarring can occur which makes the tissue feel hard. Gentle massage over the buttock area helps even out and smoothen these areas during the recovery phase.

 

Q-61 Is there any major risk within buttock implant procedures?

 

A-      There are some rare and severe complications reported in the cosmetic surgery world. They are:

 

Infection: Infection may happen after any surgery and may rarely occur after liposculpture. The rate of infection increases with an increase in the amount of injected fat, as the risk of Seroma formation increases; but the overall rate still remains very low due to routine use of peri-operative antibiotics. There have been reports of a significantly increased risk of infection with the use of buttock implants, which can be as high as 25-30%. This is most likely due to the vicinity of the incision to the rectum. If a mild infection develops it might be treated with antibiotics. Otherwise, the implant has to be removed, and the wound has to be left open for it to close gradually, which can take several weeks. Extremely rarely, infections may be serious or life threatening such as in cases of necrotizing fasciaitis (bacteria that eat away at the tissue) or with toxic shock syndrome, a serious, sometimes fatal infection caused by a bacteria, that is associated with surgery.

 

Embolism: This is also extremely rare and is lessened by early postoperative mobility so that blood does not have time to become stagnant in calf muscles. Fat embolism may occur when fat is loosened and enters the blood through blood vessels ruptured (broken) during liposuction. Pieces of fat get trapped in the blood vessels, gather in the lungs, or travel to the brain. The signs of pulmonary emboli (fat clots in the lungs) may be shortness of breath or difficulty breathing. If you have the signs or symptoms of fat emboli after liposuction, it is important for you to seek emergency medical care at once. Fat embolism may cause permanent disability or, in some cases, be fatal.

 

Visceral Perforations (puncture wounds in the organs): During liposuction or fat injection the physician is unable to see where the cannula, or probe, is. It is possible to puncture or damage internal organs during this procedure. When organs are damaged, surgery may be required to repair them. Visceral perforations may also be fatal.

 

Skin Necrosis (skin death): The skin above the fat grafted area or at the site of stab wound incisions may become necrotic or “die.” When this happens, skin may change color and be sloughed (fall) off. Large areas of skin necrosis may become infected with bacteria or microorganisms.

 

Implant Protrusion: It is possible for the buttock implants to migrate and gradually extrude through the incision or other areas of the buttock skin. The incidence of this increase if there is Seroma, Hematoma, or Infection. Also, excessive pressure over the implants during the early period of healing can increases the risk of implant protrusion.

 

Nerve Injury: Buttock implants can either press upon or migrate onto the sciatic nerve which travels in the buttock region. This can potentially cause impingement or injury to the nerve. Treatment may include removal of the implants.

 

Fluid Imbalance: Fat tissue, which contains a lot of liquid, is removed during liposuction. Also, physicians may inject large amounts of fluids during liposuction. This may result in a fluid imbalance. While you are in the physician’s office, surgical center or hospital, the staff will be monitoring you for signs of fluid imbalance. However, this may happen after you go home and can result in serious conditions such as heart problems, excess fluid collecting in the lungs, or kidney problems as your kidneys try to maintain fluid balance.

 

Toxicity from Anesthesia: Lidocaine, a drug that numbs the skin, is frequently used as a local anesthetic during liposuction. You may have had a similar drug, Novocaine, to numb your mouth at the dentist. Large volumes of liquid with lidocaine may be injected during liposuction. This may result in very high doses of lidocaine. The symptoms of this toxicity are lightheadedness, restlessness, drowsiness, tinnitis (a ringing in the ears), slurred speech, metallic taste in the mouth, numbness of the lips and tongue, shivering, muscle twitching and convulsions. Lidocaine toxicity may cause the heart to stop, resulting in death. In general, any type of anesthesia may cause complications and is always considered a risk during any surgery.

Q-72   Is it better to go on a diet before having Liposuction/Liposculpture?

 

A-        It is useless to go on an extenuating diet before having Liposuction, to then, perhaps, put on weight in the following months. We recommend that our clients have the surgery when they think they have reached the weight they can maintain. In most cases, however, after Liposuction, a series of psychological mechanisms are set in motion which lead clients to be more careful about the way they eat and in taking care of their bodies, so that in the months after surgery, they often experience weight loss and an improvement in their physical condition. Q-73  How much weight can you lose with Liposuction/Liposculpture?

 

A-      The scope of Liposuction is not the loss of weight but the remodeling of the body. It is, generally, carried out on two well defined areas and not on the entire body. It is not a drastic solution but a procedure which, if performed properly, involves very few risks with a gradual and not uncomfortable convalescence. Generally speaking, unless there is a wish to run unnecessary risks, during the course of the procedure, not more that 5-6 liters (liters, and not kilos because the fat weighs very little) is removed. It is ideal to have the surgery after a diet and, in any case, once you have reached the weight under which you do not wish to go.

 

Q-74   Can Mesotherapy eliminate cushions of fat?

 

A-      Mesotherapy does not usually have any effect on cushions of fat. This type of treatment is none other than a way to convey trophic-effect medicines to fatty tissue. It may also have some effect on (lipodstrophy) but the therapy has to be followed with perseverance (it is never definitive), having it out together with a diet.

 

Q-75   Can Liposuction of the abdomen endanger a pregnancy in any way?

 

A-      No. There are no problems in being pregnant after a Liposuction of the abdomen. The small scars are final but, if treated properly (correct suturing, massage, lymphatic drainage), they are usually not very evident.

 

Q-76   How long after giving birth can you undergo Cosmetic Surgery?

 

A-      The time that should pass after a birth before undergoing Cosmetic Surgery varies and depends on both the kind of surgery and the general condition of the client which may be different according to the case. With regard to Liposuction, the procedure can be performed after 3 months if you are not breast feeding.

 

Q-77  In what period of the year is it best to have Liposculpture?

 

A-      Even though the surgery can be performed at any time during the year, the cold months are to be preferred mainly because after the procedure the client should not be exposed to the sun for at least a month and because, when it is hot, it is uncomfortable to wear a compression garment. If these factors cause no problems, Liposuction/Liposculpture can also be performed during the summer.

 

Q-78   Many argue that a local anaesthetic is safer than a general (or total) anaesthetic. Why?

 

A-      Local anaesthetic is safe if the dosage is not excessive. When the area to aspirate is extensive, the high doses of local anaesthetic required are toxic and may cause serious complications. In these cases, a general anaesthetic (by inhalation) guarantees the best results, reducing risks to a minimum. In any case now, after a few hours, in most cases, the client can return home.

 

Q-79   On what basis is the decision made to operate under local or general anaesthetic?

 

A-        The decision depends mainly on the number of areas to be operated on, the amount of fat to be removed from each single area and on the weight of the client. The dose of anaesthetic that could be toxic varies, therefore, according to the case. Most procedures (of a small or medium entity) can, nevertheless, be performed in day hospital under local anaesthetic with sedation. If the fat to be removed is over 2.5 litres, the advisability of operating under general anaesthetic will be considered.

 

Q-80  Some argue that to obtain good results, Liposuction/Liposculpture should be performed under local anaesthetic with the client in a standing position so that the fatty cells to be removed can be seen better. Is this true?

 

A-      The cosmetic surgeons in most clinics do not agree. With Liposuction/Liposculpture, it is not always possible to keep a client standing up: blood pressure tends to go down and they could faint. The surgeon must evaluate this. Furthermore, with advanced techniques like superficial Liposculpture, after the fat has been removed, if you are still on your feet, the skin appears to be sagging. The result looks terrible and completely different from that which will be the final outcome after the final modeling and compressive medication carried out by the surgeon.

 

Q-81   What is a local anaesthetic with sedation?

 

A-      So-called sedation involves the intravenous administration of drugs which diminish or nullify the discomfort related to surgery. It involves substances that are anxiolytic (for reducing tension), hypnotic (for making you sleepy), analgesic (for killing pain). Finally, some substances induce amnesia (you do not remember almost anything about the surgery). If the client so wishes, it is, nonetheless, possible not to have any sedation.

 

Q-82  What is the difference between Liposuction, Liposculpture and Lipoaspiration?

 

A-      Liposuction and Liposculpture are quasi synonyms. At present, for Liposculpture, we mean a Liposuction with a multiplanar approach which also includes superficial Liposuction and the use of fat for refilling sunken areas (Lipofilling or Lipostructure), to obtain a global remodeling of the areas to be treated.

 

Q-83   What are the possibilities of having Liposculpture on the calves and ankles?

 

A-      Lipoaspiration of the calves and ankles is possible and, if the conditions are right, you can achieve good results; if the volume of the calves is caused by muscles and tendons or bones, very little can be done. Water retention is not a contraindication but, after the procedure, it may continue and, therefore, affect the validity of the end result. In this region, post-operative swelling lasts much longer than in the other areas. For this reason, it is advisable to have the surgery in the winter months and then have Lymphodrainage treatments.

 

Q-84  If you have problems with your circulation, can you still have Liposuction/Liposculpture?

 

A-      Yes, the surgery can be performed in most cases with a good outcome, both aesthetically and physically. The type of surgery must, however, be analysed during the course of the preliminary consultation.

 

Q-85  Is it necessary to stop taking the pill before Liposuction?

 

A-      It is advisable but not obligatory. Women who take the pill need to take a course of anticoagulant drugs (deriving from heparin), as preventive medicine, both before and after surgery.

 

Q-86   What are the percentages of risk with Liposuction/Liposculpture? Is it true that they are now considerably less?

 

A-      The risks of Liposuction/Liposculpture in healthy people are those common to all surgical procedures (reactions to the anesthetic, infections, etc.) but complications are very rare. It is, however, necessary to operate with the proper precautions: suitable clinics, assistance with the anesthetic even in procedures performed under local anesthetic, the use of a general anesthetic in more extensive Liposuctions (giving too much anesthetic risks being toxic). The swelling technique and the use of fine cannulas guarantee the best results with very little bleeding.

 

 

Q-87   Isn’t there the risk that, where the fat has been removed, the excess skin will sag?

 

A-      For this reason, the Cosmetic Surgery specialists in many clinics prefer Liposculpture, that is, superficial Liposuction which, apart from reducing the cushion of fat, increases the skin tone. Moreover, the surgeon has to take into account, at least generally, whether the skin will be elastic enough to adapt to its new condition

 

Q-88   What is Lymphodrainage?

 

A-      Lymphodrainage is a special massage which aids in the re-absorption of pre- and post-operative swelling thereby speeding up recovery. It can also be performed, with a great many advantages, with the LPG – Endermologie method. It must be done by expert personnel who have taken special courses.

A-   Cosmetic Harmony recommends sending your entire medical test and ask all your questions before making reservations for your travel to Colombia. This means sending all photos requested by the doctor as well as being 100% honest with any pre-existing conditions you may have such as diabetes or high blood pressure etc. Having pre-existing conditions does not necessarily mean you will not be able to have surgery. In most cases we will be able to accommodate you however special treatments and medication may be needed (this conditions may increase the total cost). We recommend getting your blood work done in your home town as to avoid traveling to Colombia and having an issue arise that does not allow you to undergo surgery. A-   In Colombia, the law requires all patients to sign an agreement to state that he/she will undergo surgery on his/her own free will.

If for some reason you are not able to make a decision, the physician will discuss the matter with the person you have assigned as your guardian.

That will not be a problem as we do offer additional time in our facilities to recuperate. If more time is needed, it is $100 US per day. This does not include additional medication or doctors appointments if necessary.
